nCourage is a collaborative service hub for at-risk individuals, victims, and survivors of sexual exploitation. The program provides a safe, centralized access point for youth 12-24 to access to receive services and supports for victims and survivors. The goal of nCourage is to improve access to services and support for victims and survivors in one safe location and across Peel.

Position Profile:
Reporting to the Client Services Manager, the Client Navigator provides trauma and violence informed, client-centered case management to vulnerable youth aged 16–24 within the Region of Peel. Working from an anti-oppressive, and harm reduction framework, the Client Navigator supports youth in achieving individual goals and long-term well-being through assessment, planning, facilitation, care coordination, engagement, evaluation, and advocacy.
The Client Navigator supports a strengths-based approach, conducts assessments, utilizes care planning tools, facilitates care coordination, and advocates for options and services to meet the youth’s comprehensive needs in order to achieve greater life stability. This position supports youth in identifying barriers, setting goals and accessing appropriate resources to foster connection, belonging, and independence.
The position requires strong collaboration with community partners, law enforcement, and specialized Anti-Human Sex Trafficking services to ensure safety, permanency, and well-being for children and youth suspected or entrenched in human sex trafficking.
This position works both on-site, and in the community, flexible working days, and evenings, and weekends as assigned. The role involves responding to clients in crisis and coordinating with nCourage partners, emergency services and community partners both onsite and in the community.
Role Overview:
Responsibilities:

Conduct assessments to determine client needs
-
Develop and implement culturally appropriate interventions to help youth develop actionable goals
-
Manage individual progress of youth case plans and follow-up to ensure progress towards goals is being made; adjust goals as necessary in conversation with the youth
-
Keep careful and accurate records and maintain client case file of all relevant information pertaining to assigned caseload, via the designated database.

Conduct therapeutic, trauma and violence-informed risk assessment and safety planning.

Support identifying the client’s needs, and desired referrals to on-site and off-site partners for service.

Collaborate with partners to create a seamless continuum of care and access to services.

Coordinate and develop the onsite calendar of service partners for clients to access.

Participate in care conferencing and consultations with nCourage team, and/or partner organizations.

Consult and report any risk, and/or concerns to appropriate leadership/agencies.

Coordinate in person services, ensuring appointments are confirmed and coordinated daily as part of the continuum of care for clients.

Participate in and facilitate committee meetings, as assigned.

Advocate for clients and report barriers and gaps as identified by clients.

Coordinate with internal and external partners to help clients secure affordable and appropriate housing using a Housing First for Youth model and to ensure wraparound services are being offered, based on assessment of needs and goals. This may include, but are not limited to:

Health care providers

Mental health and wellness services
-
Addictions supports
-
Ethno-specific services and supports
-
Housing and landlord relationships
-
Aftercare supports
-
Youth-specific supports that may include employment, education, connection with community, and more

Other duties as assigned.

Accompany youth to viewings and meetings when necessary and act as an advocate/support, in consultation and collaboration with the Transitional Housing Support Program.

Demonstrate skill and situational awareness when engaging and/or in the community with youth to maintain appropriate safety precautions.
Other Responsibilities:

Compile statistical information and prepare reports as required for program evaluation.

Support client data collection by regularly auditing and review data. Maintain detailed and appropriate documentation, record keeping, progress notes, and care path plans, within assigned databases.

Work collaboratively with community partners to support client’s needs.

Represent Our Place Peel and nCourage professionally at community events, initiatives, and partnerships.

Attend group programming and presentations for community partners, or those at-risk. As assigned.
Qualifications:
-
Bachelor's degree in social work, or related human service field, or equivalent combination of education and/or experience.
-
2-3 years of relevant work experience with victims of human trafficking.
-
Demonstrated knowledge of human trafficking, gender-based violence, experience working with victims and survivors of human trafficking, and trauma.

Knowledge of the principles and practices of counseling, social work, and case management.

Demonstrate positive interpersonal skills and ability to work in a team environment with co-workers, community providers and stakeholders, and clients.

Strong organizational and time management skills, ability to prioritize and meet multiple deadlines.

Excellent verbal and written communication skills in English (French as well as other languages are considered an asset).

Proficient in Microsoft Office Suite and working knowledge of database management.

Ability to work independently.

Satisfactory Vulnerable Sector Police Check and valid CPI/UMAB and First Aid/CPR certifications.

A valid Ontario driver’s licence and access to a vehicle with a $2,000,000 (minimum) liability car insurance coverage are mandatory.
