For 40 years, the Alzheimer Society of Sarnia-Lambton has supported individuals and families affected by dementia across our community. Through education, support programs, and system navigation, and in partnership with local healthcare and community organizations, the Alzheimer Society of Sarnia-Lambton plays a key role in ensuring people receive the care and connection they need.
The Alzheimer Society of Sarnia-Lambton (ASSL) is a community support service organization that provides programs and services to people living with dementia and their care partners in Sarnia-Lambton. We work closely with other service providers to ensure quality care and support for our clients.
We employ administrative and fund development staff, educators, social workers, nurses, therapeutic rec staff, personal support workers and volunteers who work together as a team to meet the needs of people living with dementia and their care partners.

What You Will You Be Doing
Ongoing Client Support:
· Provide intentional ongoing support and follow up to those referred by the First Link Care Navigator
· Keep an accurate and up to date client case load for active monitoring
· Continually monitor and assess needs of clients on the case load
· Refer internally or externally as necessary to address the needs of clients on the case load
· Coordinate services to address the needs of clients on the case load
Counseling:
· Assist with development of Counseling policies and procedures
· Provide social work services, including bio-psycho-social assessments focused on support, education and case management for individuals, families and groups affected by Alzheimer’s disease or related dementias
· Provide service via group sessions, home visits, office visits, telephone and email
· Provide outreach for multicultural, rural and other underserved communities
· Assess and respond to high-risk situations with potential for abuse, family violence, and suicidal behavior
· Consult and liaise with other disciplines and community partners to establish appropriate intervention plans and referrals
· Advocate for client/family’s identified needs and services within the agency and the community
· Support and promote inter-agency teamwork and collaboration
· Develop and maintain effective relationships with community partners
· Facilitate support groups
· Participate in professional and community education and research activities, including committee membership
· Adhere to OCSWSSW standards and ethics
· Maintain accurate client records in accordance with OCSWSSW standards
· Monitor, document, and evaluate social work activities and relevant literature
· Maintain and submit statistics, as required by funders
· Conduct evaluation of practice using measurable outcomes
Pro-active Follow-Up:
· Provide supports to care partners as the person they support transitions through use of different parts of the health, social and residential care systems
· Monitor and provide proactive follow-up for care partners
Service Delivery Standards and Quality Improvement:
· Maintain confidential, accurate and current client records, including complete and thorough documentation for each client contact, in compliance with relevant privacy legislation and in accordance with professional standards and internal policies
· Ensure that client consents, privacy, and confidentiality are maintained in compliance with legislation, professional standards/regulations, and internal policies
· Maintain an advanced level of knowledge of Alzheimer’s disease and other dementias, including clinical manifestations, behaviours, current care practices, treatment options, placement options, available community resources, and all relevant legislation
· Assist with the development and maintenance of policies, procedures and resources to support First Link referrals, intake, system navigation, care coordination, and follow-up activities
· Participate in knowledge transfer and exchange and collaborate with Alzheimer Societies across Ontario to support the delivery of best practices and ongoing quality improvement
Monitoring/Evaluation:
· Collect, maintain and report required quantitative and qualitative data to support province-wide monitoring, evaluation and reporting
Group Program Facilitation:
· Lead group programs for family care partners of persons living with dementia (e.g. First Link Learning Series, Enhancing Care, Support Groups, Psychoeducational Groups)
· Contribute positively to client care through the application of research-based practice
· Plan and implement program outreach and promotional events with local community dementiaprogram service providers
· Contribute to creation, evaluation and publication of client and/or program promotion materials
· Participate in program training, mentoring, and professional development activities
Other:
· Maintain and submit program statistics as required
· Supervising, mentoring, and evaluating students during their placements to ensure they meet their learning objectives.
· Perform other duties consistent with the job classification, as required
· The Social Worker generally completes all duties within normal business hours; however, flexibility in hours is required, with occasional evening and weekend work.
· Participation in the respite on-call rotation (phone advice) is a requirement of this position.
Education:
· Post-secondary or graduate degree in Social Work from an accredited university with current registration with the Ontario College of Social Workers and Social Service Workers.
· Further study in the field of gerontology, dementia is an asset.
Experience:
· 1 to 3 years client service experience in the health and/or social service sectors
· Experience working directly with people living with Alzheimer’s disease or other dementias and their care partners
· Experience and knowledge in management of chronic and complex health conditions
· Experience in assessment and care planning/coordination
· Ability to conduct needs and risk assessments
· Knowledge of available community services/supports and clinical, social and residential care options
· Understanding of roles and linkages across primary care, community care and specialized geriatric services
· Experience working in settings requiring inter-professional collaboration
· Strong knowledge of client-centered philosophy
· Knowledge of clinical practices and training models related to dementia (e.g.: P.I.E.C.E.S. and U-First!)
Other Knowledge, Skills, Abilities or Certifications:
· Registration with the Ontario College of Social Workers and Social Service Workers (OCSWSSW)
· Knowledge and adherence to OCSWSSW Code of Ethics
· Knowledge and understanding of all pertinent legislation and policies, including the Personal Health Information and Protection Act (PHIPA, 2004)
· Knowledge of case management, case conferencing and intervention strategies
· Competency related to diversity and oppression
· Excellent organizational, verbal and written communication skills
· Exceptional interpersonal skills, including shared decision-making and facilitation
· Ability to prioritize workload and manage competing tasks
· Ability to take initiative and be resourceful
· Excellent problem-solving and change management skills
· Proficiency in technology (e.g.: Microsoft office and case management and care coordination systems)
· Demonstrated ability to work independently and within a team
· Expertise and experience in cultural sensitivity and diversity
· Ability to speak French or other languages is an asset
Travel Requirements:
· Frequent travel in and around Lambton County to carry out the functions of this position, and to attend relevant meetings. Occasional travel outside of Sarnia-Lambton may be required. Valid driver’s license and access to a dependable vehicle are required
Physical Demands:
· Performance of general office duties
· Some lifting and transferring of material (display banners, pamphlets, booklets etc.)
· Significant periods sitting at the computer, but with the opportunity to move away from theworkstation
· Sensory attention is required for the majority of the workday (looking at computer screens, reading documents, etc.)
The posted salary range reflects the full compensation band for this position. Most candidates who meet all qualifications and are fully prepared to perform the role independently can expect an offer near the midpoint of the range.
Flexibility: We have a flexible work arrangement policy that allows employees to work from home or telecommute based on their job role and job tasks available to staff members after 6 months of service.
