The Gwa'sala-'Nakwaxda'xw Nation is seeking a compassionate, motivated, and community-focused Maternal Child Home Visitor to join our Health & Family Services team on a one-year temporary leave coverage basis.
The Home Visitor plays an important role in supporting the health and well-being of children and families by delivering maternal and child health services, promoting healthy child development, providing education and advocacy, and connecting families with community resources. This position works closely with the Maternal Child Health Nurse and other members of the Health team to strengthen family wellness and support healthy communities.
Key Responsibilities
Reporting to the Maternal Child Health Nurse, Health Manager, and Director of Health, the successful candidate will:
- Deliver approved Maternal Child Health services through home visits, clinic visits, workshops, and community programming.
- Assess the health needs of children and families and assist in developing individualized health plans.
- Support prenatal and postnatal care in collaboration with the Maternal Child Health Nurse, Community Health Representative, and Community Health Nurse.
- Plan and facilitate parenting groups, educational workshops, and activities that promote healthy child development and injury prevention.
- Assist families in accessing health services, screening tools, assessments, and community supports both on and off reserve.
- Promote healthy lifestyles, disease prevention, and family wellness through education and outreach.
- Encourage healthy home and community environments.
- Advocate for culturally appropriate services and assist families in navigating available resources.
- Promote client self-advocacy, empowerment, and holistic wellness.
- Maintain confidentiality and adhere to all Gwa'sala-'Nakwaxda'xw Health & Family Services policies, procedures, and Code of Ethics.
Qualifications
The successful candidate will possess:
- Grade 12 graduation or equivalent.
- Strong interpersonal and communication skills with the ability to work effectively as part of a multidisciplinary health team.
- Knowledge of child development, community health, and family wellness.
- Knowledge of community development, social and political systems, First Nations history, and Indigenous health issues.
- Strong critical thinking, problem-solving, and organizational skills.
- Ability to adapt professional knowledge and skills to a community-based setting.
Preference will be given to qualified applicants of Indigenous ancestry, with additional preference given to qualified Gwa'sala-'Nakwaxda'xw Nation members.
Requirements
- Valid BC Class 5 Driver's License.
- Reliable vehicle.
- Satisfactory Criminal Record Check with Vulnerable Sector Screening.
- Current First Aid/CPR Level C certification, or willingness to obtain.
- Willingness to participate in cultural safety and trauma-informed practice training.
