TransCare Community Support Services is a charitable not-for-profit corporation dedicated to providing home and community support services for people who need assistance due to illness, convalescence, disability, or functional limitations related to aging. TransCare® offers a continuum of services to facilitate independent living, comfortably and safely in the clients’ homes, and in the community.
Position Overview:
Responsible for planning and providing community-based, social and recreational programs to seniors and adults with disabilities, to improve and maintain their well-being (physical, cognitive, social and emotional), to enhance their independent living skills and overall quality of life.
Primary Duties & Responsibilities:
- Coordinate and support the delivery of social dining programs across multiple sites, working closely with staff, volunteers, students, and internal/external partners to ensure consistent, high-quality service.
- Plan, facilitate, and evaluate social, recreational, and wellness-based activities (in-person and virtual) that promote meaningful social connection and support seniors’ physical, emotional, and social well-being.
- Conduct regular client wellness check-in calls and follow up on client inquiries, including new referrals, intakes, and assessments.
- Track client participation, monitor engagement and progress, and maintain accurate client records, including updating client files and entering data required for program and client evaluations.
- Manage client accounts on a case-by-case basis, including client payments, scheduling, and care plans.
- Advocate for appropriate program fit, identifying when client needs exceed program scope, and facilitate referrals and linkages to additional internal and external services as needed.
- Troubleshoot and resolve day-to-day operational issues related to service quality, client concerns, staffing, or site operations.
- Ensure client safety, responding effectively to emergencies, initiating incident response procedures, and completing required follow-up and reporting.
- Orient, supervise, and support placement students and volunteers, providing guidance and oversight to ensure positive learning and service experiences.
- Work collaboratively with program site staff to ensure compliance with all organizational policies, safety standards, and required guidelines.
- Research, coordinate, and engage external vendors, entertainers, and guest speakers to enhance program offerings and special initiatives.
- Plan, monitor, and maintain program-related expenses, ensuring responsible use of resources.
- Complete a range of administrative duties, including program scheduling, preparation of print and promotional materials, data tracking, and reporting.
- Perform additional tasks and duties as assigned or deemed necessary by the program supervisor.
Job Specifications: (Minimum Knowledge/Education, Experience, Skills and Abilities normally required for competent performance in the job)
- Post-secondary degree or diploma (preferably Activation Coordination/Gerontology, Recreation and Leisure Services); or equivalent extensive experience in community service sector.
- 3-5 years front line working experience, with sound knowledge on services related to the elderly and people with challenges.
- Working experience in the mental health environment is an asset.
- Genuine interest in people, empathetic, compassionate and sensitive to the cultural characteristics of clients from various ethnic groups.
- Strong communication skills – both written and verbal
- Sound problem solving and decision-making.
- Ability to work as a team and willingness to take initiative and manage a fast-paced environment.
- Effective and timely follow through with issue resolutions and to ensure communication across all teams.
- Maintain confidentiality and respects clients’ honour and dignity while providing services.
- Valid Ontario Driver’s License and access to a vehicle is a must.
- Familiarity with client database AlayaCare an asset.
- Proficient in using Microsoft Office suite (Excel, Word, PowerPoint, Outlook).

Work Conditions and Physical Capabilities:
- Monday to Friday from 8:30 to 4:30 with occasional evenings as required.
- General office environment.
- Significant amount of computer and telephone use.
- Can lift, carry and transport loads between 13.5-18kg (30-40 lbs) from Main Office to Dining Program sites.
- Standing for long periods.
