The MMHART Worker is responsible for providing residents of the Grey and Bruce counties with serious mental illness, or those at risk, with telephone or face to face crisis assessment, intervention and resolution with the goal of reduced admission or re-admission and utilization of hospital inpatient services.
Shifts are 12.5 hours in length and include overnight, days, weekend and holiday shifts. Interactions with clients in crisis. May travel with police in cruisers, wearing police vests. Environments can be unstable and uncontrolled. Multi demand work environment.
KEY DUTIES - RESPONSIBILITIES
Ø Provide onsite response crisis intervention to people in crisis in collaboration with first responders, health professionals, schools or community stakeholders
Ø Attend rural hospitals in Grey and Bruce to conduct crisis assessments
Ø Attend community locations with police for crisis intervention and/or risk assessment as a first response
Ø Preform proactive duties alongside police
Ø Attend community locations with EMS for crisis intervention and/or risk assessment as a tiered response
Ø Attend schools to provide onsite assessments without police presence, as per the Bluewater School Board suicide prevention strategy, and in an effort to avoid having youth needlessly taken to hospital for risk assessment.
Ø Provide crisis follow-up counselling for up to thirty days to ensure appropriate linkages are made to services and supports to improve client coping skills
Ø Act as a resource to other services in Grey and Bruce and liaise with Schedule 1 hospitals
Ø Provide consultation to community stakeholders, offer referrals and resources for mental health and addictions services
Ø Provide client assessment, intervention and counselling for mental health and addictions
Ø Collaborate in the multi-disciplinary approach to service
Ø Meet professional standards according to the relevant professional code of ethics
Ø Maintain current and accurate client records as prescribed in the above noted manual
Ø Maintain statistical records associated with caseload functions according to agency guidelines
Ø Meet with supervisor for general supervision, performance evaluations and identify training needs
Ø Responsible for ongoing professional development/continuing education towards the delivery of mental health services
Ø Ensure that all confidential program/caseload information and material is handled in a confidential manner
Ø Assess clients’ immediate and ongoing needs
Ø Assist clients to identify and advocate for their civil and legal rights
Ø Facilitate access to other services in areas such as assistance with daily living crisis intervention and treatment
Ø Provide clinical counselling and information to reduce the incidence, duration and intensity of a crisis in conjunction with hospital staff and first responders to a crisis
Ø Work collaboratively with other services, supports and resources, with respect to diversity
Ø Advocate for clients’ needs within the medical care and mental health systems
Ø Participate in case conferences
Ø Evaluate the achievement of goals and client satisfaction

What you will bring
Ø A university degree in a relevant program
Ø Experience working with clients with serious mental illness and/or addictions
Ø Valid driver’s license and ability to travel within Grey Bruce counties to provide mobile crisis intervention
Ø Awareness of mental health services, resources and agencies within Grey/Bruce County
Ø Ability to work both independently and as a member of a multi-disciplinary team
Ø Knowledge of psychiatry, psycho-social rehabilitation, mental health policy, DSM V and the Ontario Mental Health Act
Ø Knowledge of and experience working with police services
Ø Excellent communication, interpersonal and networking skills
Ø Effective decision making and crisis intervention skills
Ø Minimum $2,000,000 third party liability vehicle insurance
Ø A satisfactory police record check including vulnerable sector screening
