Reporting to the Director of Care, the RAI Coordinator is a critical role within our communities as it impacts the lives of residents.

What you’ll be doing:
The RAI Coordinator is responsible for the planning, implementation, coordination, and evaluation of the RAI Program, ensuring compliance with quality management standards, legislative requirements, and organizational policies and procedures.
You will be a welcomed member team and impact in the following ways:
- Establish, maintain, and communicate current resident assessment schedule to inter-professional team.
- Ensure RAI accuracy and compliance with coding, capping and care plans are updated.
- Review and analyze each resident’s RAI assessments to ensure accuracy and meet the residents needs.
- Monitor and analyzes RAI outputs and reports.
- Produce RAI reports for leadership use to assist in clinical and operational decision-making
- Responsible for managing the computerized data submission process and maintain accurate records in accordance with Ministry of Health and Long-Term Care (MOHLTC). Collaborate with the leadership team and support services consultants to ensure data integrity and effective data utilization.
- Monitors and analyzes RAI outputs and reports, and communicates findings with the leadership team and support services committees.
- Lead the RAI education process for all participating team members
Who You Are:
- Current general certificate of registration with a college of a regulated health profession or Ontario College of Social Workers with a minimum of three years clinical and project experience.
- Has a valid license to practice
- Successful annual completion of RAI certification as per Ministry of Health requirements for Relias-MLTC
- RAI experience/education is an asset
- Good problem solving and analytical skills
