QUALIFICATIONS
Certification/Experience/Knowledge:
- Bachelor of Social Work required
- Current registration in good standing with the Ontario College of Social Workers and Social Service Workers is required
- Minimum of two (2) years of significant experience in a hospital setting, mental health and psychiatric services, with knowledge of and experience in the coordination of community services, is required
- Current Basic Life Support (BLS) for Health Care Providers (HCP) Certificate required (completed within the last twelve (12) months)
- Current Gentle Persuasive Approaches (GPA) Certificate is considered an asset
- Demonstrated knowledge of Ontario Health atHome, Placement Coordination Service (PCS), Homes for Special Care, the Association for Community Living, and Decision Support, including their appropriate utilization, is required
- Excellent working knowledge of relevant legislation, including but not limited to the Mental Health Act and the Long-Term Care Act, is required
- Commitment to continuous learning and quality improvement
- Excellent verbal and written communication skills, with the ability to communicate effectively with patients, physicians, program staff, community agencies, and other stakeholders
- Demonstrated understanding of trauma-informed care principles and the ability to apply them in all relationships and interactions
- Proven interpersonal, negotiation, and problem-solving skills to establish and maintain positive working relationships with clients, program staff, community agencies, and other stakeholders
- Knowledge of the Northeast Region and the agencies and organizations that support clients with mental health, developmental, and cognitive concerns within the community
- Knowledge of social work treatment interventions and best practices applied in recovery-oriented, clinical, and community settings, incorporating psychosocial rehabilitation values and principles into client care
- Excellent computer skills, including the ability to prepare clear, accurate, and timely reports
- Ability to function effectively as a member of an interprofessional health care team
- Demonstrated commitment to ensuring the safety of self, fellow workers, patients, volunteers, and visitors
- Demonstrated commitment to supporting and contributing to a culture of safety and the prevention of adverse health events within the organization
- Good attendance and work record required
- Bilingualism (Advanced level English/French) is required. All unilingual candidates are encouraged to apply and may be considered if the identified French requirement has been met or there are no successful bilingual candidates. A formal French test will be conducted to evaluate the proficiency level.
