CRC Care@Home Pharmacy is currently seeking dedicated Licensed Practical Nurses to join our Home Medication Management program in Richmond. The successful candidates will be responsible for administering medications, performing patient-specific assessments, acting in acute situations, and documenting all interventions and assessment data.
Responsibilities:
- Visiting patients in the community independently (multiple patients per shift)
- Safely administering medications (orally, injections, creams, patches, subcutaneous injections)
- Performing patient-specific assessments (Blood glucose, blood pressure, heart rate, weight)
- Acting in acute situations according to pre-set company protocols
- Reporting any acute changes in patients to their primary care provider, or to the program coordinator if the primary care provider cannot be reached.
- Noting changes in behavior for mental health patients
- Documenting all interventions and assessment data
- Submitting daily reports to the nurse supervisor and/or pharmacist
- Actively participating in patients’ healthcare outcomes
- No assistance with personal care or ADL’s required
Requirements:
- Currently in good standing with BCCNM
- Valid Class 5 Driver's License
- Satisfactory proof of COVID-19 vaccinations
- A reliable vehicle
- Reference check
Work Schedule:
- Morning shifts: 8 am to 12 pm
- Evening shifts: 4 pm to 8 pm
- Each shift ranges between 4 to 5 hours on weekdays and weekends. We are offering this position on a part-time, permanent, with a minimum commitment of 3 shifts per week
Wage Offer:
The hourly wage falls between $34-$38, supplemented by a $0.50/km reimbursement for work-related travel.
