Summary To support Physician and Nurse Practitioner (NP) clinics provided by the organization, located at the First Nation Health Clinics. The Registered Practical Nurse (RPN) will provide linkages and communicate with community reception, booking clerks and other external primary care providers as follow up and to ensure client receive wholistic care. The RPN delivers supportive care with an emphasis on assessment, education, prevention, maintenance, and restoration of health for clients of all generations while working within their scope of practice. Job Duties: 1. Provide clinical support§ Provide one-on-one education to clients as directed§ Triage patients on arrival – this includes a history and vital signs§ Works within scope to provide support as directed by Physician(s) and Nurse Practitioner(s) § Inputs diagnostic testing, consults and referrals into EMR§ Review all address, demographic and allergy deficiencies in the chart for accuracy.§ Immunizations as directed§ Provide skill training to clients on self-care § Assess the clinical, dietary and educational needs of clients § Medication Reconciliation § Assist with vaccine clinics as needed§ Complete client medical records within the electronic medical records (EMR) database in an accurate and timely manner 2. Perform administrative duties in support of the organization’s goals and objectives§ Assist with the development, implementation, and evaluation of education and treatment services§ Maintain inventory of supplies and place orders to refill inventory as required§ Assist with the development and evaluation of policies, protocols, procedures and advanced directives to improve client care and/or to promote efficient utilization of resources§ Provide a safe and healthy environment for clients, and clinicians by ensuring the health and safety policies are followed§ To be culturally sensitive and respectful to the indigenous population served. 3. Perform other duties as assigned§ Engage in professional development to maintain current skills and knowledge regarding the scope of the position§ Assist in the development and implementation of the organization’s quality improvement initiatives§ Network with partner agencies to improve practices, enhance services and move forward shared initiatives relevant to the position§ Set up and assist with OTN access§ Assist with Diabetes Foot Care as required Requirements 1. Mandatory§ Current registration in good standing with the College of Nurses of Ontario§ One to three years experience in a health care setting§ Current CPR Level C and First Aid Certification§ Valid Class G driver’s license and have access to a vehicle§ Clear Vulnerable Sector Check 2. Preferred§ Experience working in First Nation communities§ Experience working within an electronic medical record system 3. Knowledge§ Knowledge of the First Nation communities of Manitoulin Island§ Knowledge of the service providers within the Manitoulin Island catchment area§ Knowledge of First Nation health issues§ Understanding and sensitivity for the Anishinaabe culture 4. Skills§ Excellent verbal and written communication skills§ Strong time management skills§ Skilled in interpersonal relations§ Proficient in Microsoft Office 5.
