Overview
We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will have extensive experience in patient care across various settings, including pediatrics, senior care, nursing homes, hospitals, and assisted living facilities. This role involves providing comprehensive medical services, managing acute and chronic conditions, and supporting patients of all ages. The Nurse Practitioner will collaborate with physicians and healthcare staff to deliver high-quality, patient-centered care utilizing advanced clinical skills and medical knowledge.
Key
Responsibilities
- Triage referrals and support patient intake processes
- Conduct virtual and in-person consultations
- Educate patients on treatment options, expectations, and outcomes
- Guide patients through treatment and care pathways
- Support the surgical consent process
- Provide post-operative follow-up care, ongoing treatment follow-up, and long-term care planning
- Provide counselling and troubleshooting related to medical devices and wound care
- Participate in minor in-office procedures, as appropriate
- Participate in functional wellness consultations
- Contribute to community education and outreach initiatives
- Develop and refine clinical workflows and standard operating procedures
- Track outcomes and contribute to program performance metrics
- Coordinate care with physiotherapy and other allied health providers
- Support the development and implementation of innovative health programs

Qualifications
- Valid Nurse Practitioner license with current certification in relevant specialties such as pediatrics, senior care, or acute care.
- Proven experience working with children, seniors, or in hospital environments; nursing home or assisted living experience is highly desirable.
- Strong knowledge of medical terminology, medication administration protocols, and case management practices.
- Proficiency with electronic health record systems like Epic and Cerner; prior medical office experience is a plus.
- Skills in pain management, dementia care, laboratory procedures, phlebotomy, and home care are advantageous.
- Excellent communication skills with the ability to educate patients effectively across diverse populations.
