General Description
This position involves providing psychosocial grief and bereavement support to residents, families, and members of the community, in consultation with the Hospice Director, the Home Coordinator, and the interdisciplinary team. The individual plays a critical role in the hospice care support team, collaborating with internal and external partners, and is responsible for data collection related to their functions, supporting research and quality improvement initiatives.
Core Job Responsibilities
Responsibilities include, but are not limited to:
· Conducting psychosocial assessments.
· Providing short-term focused counseling to residents and families.
· Connecting families to community supports and coordinating hospice care family meetings.
· Facilitating group sessions at the hospice and in the community.
· Developing and maintaining relationships with community partners.
· Participating in interdisciplinary team conferences to formulate and coordinate services and supports. The role's reach may include referrals from the Interdisciplinary team within the community.
· Assist as needed and appropriate within the scope and license with legal matters such as Power of Attorney (POA) kits.
· Engages in evidence-based research to improve psychosocial interventions and outcomes.
· Provide Education to staff on ways to support families and residents.
Adherence to relevant legislation, regulations, standards set by the OCSWSSW and the policies of Ian Anderson House.
The role also encompasses responding to inquiries about hospice, grief, and bereavement support.
Qualifications
The Successful candidate will have the following certifications and skills
· Degree in Social Work of Social, Masters Preferred.
· Registration in good standing with the Ontario College of Social Workers and Social Service Workers.
· Minimum 2 years of experience in a community-based setting providing grief, mental health, or social support services preferred.
· Strong working knowledge of the Irish Adult Bereavement Care Pyramid and related assessment tools.
· Demonstrated ability to work with diverse populations and cultural understandings of grief and loss.
· Excellent communication, facilitation, and interpersonal skills, including demonstrated competence in group facilitation and training.
· Ability to work independently and collaboratively within a multidisciplinary team.
· Knowledge of community supports and services, preferably within Oakville and surrounding areas preferred.
· Familiarity with hospice, palliative care, or community health settings.
· A satisfactory current criminal reference check (CPIC) including Vulnerable Population.
· In-depth knowledge of Ontario legislation related to substitute decisions, POA, and the Public Guardian of Ontario is essential, along with familiarity with long-term care and home/community care services.
· The candidate must possess excellent assessment, evaluation, judgment, and decision-making skills, with the ability to plan, organize, and implement both proactively and responsively.
· Attend IAH events as required.
