Responsibilities

Support Inuit patients and families in understanding and accessing healthcare services, including appointments, referrals, and follow-up care.
Assist patients with navigating complex health and social service systems
Provide practical support such as appointment reminders, transportation coordination, and accompaniment when appropriate
Advocate for culturally safe, trauma-informed, and Inuit-centered care within all areas of healthcare
Support healthcare providers in understanding Inuit culture, history, and social contexts
Address barriers related to language, culture, systemic racism, and Inuit social determinants of health
Assist with arranging interpretation supports (Inuktitut and/or English)
Assist patients and families with understanding diagnoses, treatment options, and care plans
Support informed decision-making by patient and families
Work collaboratively with Inuit Health Manager and Nunatsiavut Government, interdisciplinary healthcare teams, social workers, community organizations, and Elders/knowledge keepers
Connect clients to community-based supports such as housing, income assistance, mental health services, and traditional medicines when desired
Participate in interdisciplinary rounds, and care/discharge planning
Maintain strict confidentiality
Document interactions, referrals, and outcomes in accordance with Nunatsiavut Government and Health Authority policies

Qualifications

Minimum 2-year diploma in health or social related field with 3-5 years of experience working in healthcare, social services, or community support roles. A combination of equivalent training & experience will be considered.
Strong connection to Nunatsiavut and Upper Lake Melville communities, Inuit culture and values
Lived experience (and/or deep understanding) of Inuit health realities
Strong interpersonal, communication, and advocacy skills
Ability to build rapports with patients, families, and service providers
Understanding and compliance of confidentiality and ethical practice
Experience working in healthcare, social services, or community support roles
Knowledge of regional healthcare systems and community resources
Training in trauma-informed care, cultural safety, and patient navigation
Be open to working in various settings – Nunatsiavut office, hospitals, clinics, community settings, and/or client homes
Some travel and flexible hours may be required
Exposure to emotionally challenging situations; organizational support and supervision provided
