About Toronto Poly Clinic Toronto Poly Clinic is a modern multidisciplinary clinic serving patients across the Greater Toronto Area. Our team works collaboratively to provide high-quality, patient-centred care in a supportive and professional clinical environment. We are currently seeking a Full-Time Nurse Practitioner (NP) to join our growing team. This is an excellent opportunity for an NP who is professional, motivated, and interested in working with physicians, nurses, and administrative staff in a busy outpatient clinic setting.
Position Summary:
The Nurse Practitioner will provide comprehensive patient care within their professional scope of practice. The successful candidate will work collaboratively with clinic physicians and staff to support assessments, follow-ups, treatment planning, patient education, documentation, and continuity of care.
Key Responsibilities:
- Perform patient assessments, health histories, and clinical evaluations.
- Provide follow-up care and support ongoing patient management.
- Prescribe medications and order investigations within the NP's scope of practice.
- Review results, document clinical findings, and communicate care plans clearly.
- Provide patient education and counselling.
- Support referrals, care coordination, and communication with other health care providers.
- Maintain accurate and timely documentation in the EMR.
- Work collaboratively with clinic physicians, nurses, and administrative staff.
- Support quality improvement, workflow improvement, and patient-centred care initiatives.
- Participate in clinic programs and services as needed.
Required Qualifications:
- Current registration with the College of Nurses of Ontario (CNO) as a Nurse Practitioner / Registered Nurse in the Extended Class.
- Current professional liability protection.
- Minimum 2 years of Nurse Practitioner experience preferred.
- Strong clinical assessment, communication, documentation, and decision-making skills.
- Experience using an electronic medical record system.
- Ability to work independently while collaborating with physicians and the clinic team.
- Professional, reliable, organized, and patient-focused.
Preferred Qualifications
- Experience in a multidisciplinary outpatient clinic or private clinic setting is an asset.
- Experience or interest in chronic pain, mental health, addiction medicine, injections, IV therapy, MVA-related care, or wellness services is an asset.
