Job Overview
Elevate NWO is seeking a Registered Practical Nurse (RPN) to join our multidisciplinary team. The RPN provides nursing care, health education, and client support within our clinic, drop-in, and community programs. Working under the direction of the Nurse Practitioner, the RPN delivers client-centred care within their professional scope of practice, supporting assessment, treatment, testing, and harm reduction initiatives in both clinical and outreach settings.
Duties Overview
- Provide nursing care, treatment support, and health teaching in accordance with CNO standards and agency protocols
- Participate in client assessment, care planning, and follow-up under the supervision of the Nurse Practitioner
- Conduct point-of-care and laboratory testing (HIV, HCV, and other STBBIs) in clinic, drop-in, and community settings
- Perform venipuncture and other procedures within the RPN scope of practice
- Provide pre- and post-test counselling, and connect clients to appropriate follow-up care
- Monitor treatment adherence and report health status changes to the Nurse Practitioner
- Ensure proper specimen handling, infection prevention, and quality assurance
- Maintain accurate documentation in EMR, OCASE, and OCHART systems
- Participate in community outreach and health education events promoting harm reduction and wellness
Qualifications
- Current registration in good standing with the College of Nurses of Ontario (CNO) as an RPN
- 2–3 years of relevant nursing experience preferred (community, primary care, or outreach)
- Demonstrated experience with phlebotomy, point-of-care testing, and infection control
- Knowledge of HIV, HCV, and STBBI testing, prevention, and treatment
- Excellent communication, documentation, and organizational skills
- Commitment to harm reduction, health equity, and trauma-informed care
- Valid Ontario driver’s license and reliable, insured vehicle for work-related travel
- Vulnerable Sector Criminal Reference Check required
