Hours: 40 hours/week
Reporting to: Clinical Manager
Reporting to the Clinic Manager, the Registered Social Worker will collaborate with our Mental Health Team to provide front-line support to individuals and families accessing primary care services at IIPCT. This role requires a blend of cultural sensitivity and expertise in western social work practices to address a variety of wellness needs.
Responsibilities:
- Provide social work support to individuals and families accessing Primary Care Team services, including paperwork/forms completion, needs assessment, referral and advocacy, counseling, and crisis intervention.
- Empower FNIM individuals and families by delivering culturally appropriate social work services aimed at improving wellness and resilience.
- Collaborate with the Mental Health Educators, Mental Health Team, and other internal programs to ensure comprehensive care.
- Manage caseload targets and fulfill administrative duties, including client statistical reporting.
- Actively participate in team meetings, case reviews, and IIPCT staff meetings.
- Undertake additional responsibilities and duties as assigned.
Requirements:
- Professional membership (RSW) and master’s level education OR minimum 3 years’ direct experience as a social worker.
- Experience working with Indigenous families and knowledge of cultural-based services.
- Fluency in a local Indigenous language is an asset.
- Community work experience in areas such as poverty, housing, legal issues, family violence, and mental health.
- Training or certification in mental health, substance abuse screening, crisis intervention, counseling techniques, trauma/abuse, and supporting people with disabilities.
- Knowledge of FNIM culture, values, and history.
- Willingness to engage in Indigenous Cultural Safety Training and ceremonies.
- Computer proficiency and willingness to travel within the Deseronto area and IIPCT sites as required.
- Valid driver’s license, access to a reliable vehicle, and up-to-date immunizations.
- Satisfactory police check required.
