The Nanaimo Primary Care Network is seeking a full time (1.0 FTE) nurse practitioner to work with a team of practitioners and other health providers to collectively deliver care to patients at the Snaw-Naw-As Health Centre.
The Snaw-Naw-As Health Centre is a 4,000-square-foot facility located on the hillside overlooking Nanoose Bay, BC. The Health Centre provides Primary Care and other health services, including both modern treatments and traditional healing methods, to the Snaw-Naw-As people, of the Nanoose First Nation. Along with 18 other tribes in the Salish Sea we are Coast Salish people, one of the northern most tribes on the east side of Vancouver Island.
The Snaw-Naw-As Health Centre is home to an interdisciplinary team of providers including Physicians, Community Health Nurse, Maternal Health Worker, and several visiting service providers including other family and child supports.
The Snaw-Naw-As Health Centre operates from 0900 to 1630 Monday to Friday; however clinic hours may change based on practice needs.

The Nurse Practitioner will provide low barrier primary care to an Indigenous patient population with complex medical needs. This can include patients of various ages with multiple co-morbidities, past experiences with trauma and/or significant social and cultural barriers to care.
