The Health for All Family Health Team is seeking a dedicated and compassionate Registered Nurse to join our interprofessional primary care team. As a Primary Care RN, you will provide comprehensive nursing services to individuals and families across the lifespan — guided by RNAO Best Practice Guidelines and a commitment to health equity.
You will work collaboratively with physicians, nurse practitioners, pharmacists, and other allied health professionals to deliver patient-centred care that spans health promotion, chronic disease management, and disease prevention.
Nursing Care
- Provide ongoing assessments, counselling, screening, referrals, education and follow-up for clients of all ages within a primary care framework guided by RNAO Best Practice Guidelines.
- Collaborate with the interdisciplinary team to deliver safe, effective nursing care within your full scope of practice.
- Perform telephone triage and monitoring within the clinic setting.
- Perform technical nursing interventions including injections, dressing changes, wart treatment, ear syringing, and other procedures.
- Sterilize and maintain medical equipment.
Health Promotion & Education
- Provide health education, counselling and disease prevention to patients and families across the continuum of care — including immunization, chronic disease management, well baby and family care.
- Support Health for All’s programs including Diabetes, Smoking Cessation, Mental Health, and Complex Care.
- Participate in teaching interprofessional team members and Family Medicine Residents.
Team & Organization
- Assist physicians and interprofessional healthcare providers in day-to-day clinical management of patients.
- Communicate effectively with team members, patients, families, and community partners.
- Document care accurately using patient records and EMR systems.
- Participate in interprofessional committees, clinical projects, and studies as required.
- Promote awareness of Health for All’s services and programs.
- Maintain and develop professional competence through ongoing professional development and self-directed learning.
What You Bring
Required
- Current General Class (Practicing) Registration with the College of Nurses of Ontario.
- Current Basic Cardiac Life Support (BCLS) Certificate.
- Demonstrated knowledge of and commitment to primary health care, health promotion, disease prevention, and chronic disease management.
- Demonstrated ability to work effectively in an interprofessional team environment.
- Strong assessment and triage skills.
- Excellent interpersonal, communication, decision-making, problem-solving, conflict management, and time management skills.
- Satisfactory attendance record.
Preferred
- 3–5 years of recent experience in a primary care/family practice, community, or emergency nursing setting.
- Experience in an underserviced or community primary care setting, or combination of community, hospital, or public health settings.
- Demonstrated experience teaching health education to individuals, groups, and interprofessional learners (including residents and clinical clerks).
- Excellent computer skills with a minimum of 2 years’ experience using an EMR primary care system.
- Knowledge and proficiency in current, evidence-based methods and practices of primary care delivery.
- Acute care experience is an asset.
