Job Purpose/Summary
Functioning within the current standards of nursing practice as established by the College of Nurses, this position will work collaboratively with the Community Paramedicine Program to provide primary care to patients in the community and will liaison between various health services within the community and coordinate community wide services as well as assist with Health Promotion and Prevention as needed.
Education
- Registered with the College of Nurses of Ontario. Registered Nurse with RNAO or CPNP.
Skills and Abilities
- Knowledge of community paramedicine programs.
- Knowledge and understanding of indigenous culture and protocols and trauma informed approaches to health care.
- Excellent communication and interpersonal skills.
- Ability to multi-task in a complex fast paced environment.
- Skills in home assessment and developing patient care plans.
- Must complete Phlebotomy Course and Transportation of Dangerous Goods within first 3 months.
- Good understanding of community and regional health services.
- Ability to work independently as well as collaboratively with various community departments and regulated health professionals.
- Valid First Aid/CPR.
Experience
- Experience with or training in mental health, addictions, palliative care, advanced assessments, geriatric care, community health care, chronic disease management an asset.
- Experience in public/community health nursing, program planning, implementation and evaluation.
- Experience completing blood draws an asset.
- Experience working with First Nation communities.
- Experience using remote patient monitoring equipment an asset.

- Team player utilizing nursing practices, relevant theory and research findings.
- Develop an annual program plan in collaboration with the Paramedicine Team.
- Oversee and coordinate services and provide follow up to ensure quality of care.
- Regular home visiting and providing nursing care as required.
- Assess and document the physical, mental, social and environmental health of patients enrolled in program.
- Ensure accurate documentation of patient assessments, plan of care, care provided etc.
- Provide initial client assessment and make referrals to appropriate agencies and healthcare providers.
- Examine, assess and screen individuals to coordinate and initiate follow up with existing medical and other health care services and appointments with various health care providers as needed.
- Collaborate to ensure an integrated approach to care with other departments, i.e., Home & Community Care, Social Services, Primary Care, Community Health Care Team.
- Teaching treatment procedures and care techniques to patient and families.
- Provide guidance to patients related to diabetes prevention and treatment, hypertension, congestive heart failure, chronic obstructive pulmonary disease, fall assessment, injury evaluation, geriatric frailty visits and self-neglect assessments and nutrition.
- Prepare reports on issues involving services to include direct contact and support, quality improvement and quality assurance.
- Assist in emergency first aid as required and provide follow up as needed.
- Develop and deliver a variety of educational and health promotion sessions to the community.
- Participate and assist with wellness clinics, vaccination and testing clinics.
- Maintain client confidentiality.
- Weekly blood collection clinics.
- Assist with remote patient monitoring client care and follow-up.
- Documentation of all client notes using Prehos and EMH-Ware.
- Prepare monthly, quarterly and annual statistical reports.
