Prescare is seeking a compassionate, dependable LPN to provide individualized nursing and personal care support in a client's home. Our clients benefit most from consistency — familiar caregivers, steady routines, and calm, confident communication. We're looking for an LPN who can build trust over time, tune in to individual client cues, and provide reliable, ongoing support as part of a small, close-knit care team.
Responsibilities
- Provide safe, respectful, person-centred nursing care in the client's home
- Monitor the client's condition and recognize changes requiring intervention or escalation
- Respond appropriately to medical emergencies, including seizure activity, following the client's prescribed care and emergency-response plans
- Assist with personal care, hygiene, positioning, mobility, and daily routines per the care plan
- Maintain the client's dignity, comfort, safety, and established routines
- Document assessments, care, medications, interventions, and outcomes each shift
- Communicate changes or concerns promptly to family and to Prescare
- Collaborate respectfully with family and other care professionals
What you need
- Active registration with ANBLPN and in good standing
- Current BCLS certification
- Minimum 1 year of experience in acute or chronic care, including exposure to complex care needs (e.g., seizure management, disability support, or home care)
- Competence in medication administration and emergency response
- Clear criminal record and vulnerable sector checks
- Valid driver's licence and reliable vehicle
- Strong observation, documentation, and communication skills
- Comfortable working independently in a family home setting
What we offer
- $34.75–$39.19/hour, based on experience
- Mileage reimbursed for client-related travel during your shift
- Starting at 5–10 hours/week, with a plan to reassess hours based on the client's care plan and evolving needs
