Position Overview
We are seeking a dedicated, empathetic, and energetic Transitions Case Manager. In this hands-on role, you will provide individualized, relationship-based support to youth and young adults transitioning out of care. Following the philosophy of "Connection First," you will build trusted relationships to help clients develop independence, build resilience, and successfully navigate the path to adulthood.
Key Responsibilities
- Authentic Engagement: Connect with youth using inclusive, youth-centered approaches that respect diverse identities and experiences. [1]
- Specialized Support: Provide tailored guidance to vulnerable youth, including those experiencing homelessness, pregnant or parenting youth, and individuals facing mental health, substance use, or developmental challenges. [1, 2, 3, 4, 5]
- Life Skills Coaching: Mentor and teach practical daily skills, including budgeting, cooking, apartment searching, job interview prep, personal hygiene, and building healthy boundaries. [1]
- Accompaniment & Transit: Provide direct transportation or help youth navigate transit systems to attend essential appointments and community activities. [1, 2]
- Community Collaboration: Partner with local landlords, employers, schools, and health services to build a strong network of support for your clients. [1]
Qualifications & Requirements
- Education: University Degree or Diploma in Social Work or other Social Sciences, OR a College-Level Certificate in Human Service Practitioner, Child and Youth Care, or Social Services Community Worker.
- Language: Fluently bilingual in both English and French (written and spoken).
- Experience: Proven experience working directly with a vulnerable youth population.
- Approach: Deep understanding of trauma-informed care, harm reduction, and culturally responsive practices.
- Flexibility: Ability to work non-traditional hours, including evenings and weekends, to meet youth needs.
- Assets: A valid driver’s license and access to a reliable vehicle are highly required for this role.
- Screening: Must clear a Criminal Record Check and Vulnerable Sector Screening.

Pay: $24.50-$25.00 per hour
