Job Summary
We are seeking a dedicated and experienced Nurse Practitioner to join our healthcare team. The ideal candidate will provide comprehensive patient care across various settings, including pediatrics, senior care, and acute care environments. This role involves working closely with patients of all ages, managing complex health conditions, and collaborating with multidisciplinary teams to deliver high-quality healthcare services. The Nurse Practitioner will utilize their expertise in medical terminology, medication administration, and case management to ensure optimal patient outcomes. Prior experience in nursing homes, hospitals, assisted living facilities, or home care is highly valued.
Duties
- Conduct thorough patient assessments and develop individualized care plans
- Diagnose and treat acute and chronic illnesses across diverse patient populations
- Administer medications and perform procedures such as pap and laboratory testing
- Manage pain and dementia-related conditions with appropriate interventions
- Collaborate with physician and other office to coordinate patient care
- Document clinical findings accurately using electronic health records system - Practice Solution
- Provide patient education on health management, medication use, and preventive care
- Assist with case management for patients requiring long-term or complex care solutions
- Support patients in assisted living, nursing homes, or home care settings as needed
Skills
- Proven experience working with children (pediatrics) and senior populations
- background in nursing home, hospital, or laboratory environments
- Expertise in acute care, pain management, dementia care, and home health services
- Proficiency in medical terminology and medication administration protocols
- Familiarity with electronic health record systems such as PS
- Excellent patient care skills with a compassionate approach to diverse healthcare needs
- Ability to perform laboratory procedures including pap etc
- Effective case management capabilities for complex or long-term patient needs
- Knowledge of medical office operations and documentation standards in medical settings
