We are looking for Clinical Pharmacists who enjoy collaborating with Physician Specialists and other healthcare professionals. Treo staff members enjoy the satisfaction of working in a supportive team-centric environment that delivers an exceptional patient care experience.
Roles and Responsibilities:
- Collaborate with physician specialists to initiate treatment plans and facilitate monitoring and access to pharmacotherapy treatments
- Participate in interdisciplinary case conferences to amalgamate recommendations between different specialists
- Engage with patient support programs and dispensing pharmacies on behalf of physicians and/or clinics to help coordinate pharmacotherapy.
- Utilize Electronic Health Record (EHR) software to document relevant clinical notes and identify relevant clinical factors for each patient’s treatment
- Assist with the necessary documentation affiliated with drug access navigation, medical clearance, and standing orders where appropriate to provide efficient care for a patient.
- Conduct reviews of relevant medical history as it pertains to a patient’s chief complaints
- Provide education to patients regarding pharmacotherapy, treatment plans, and additional monitoring where relevant.
- Maintain confidentiality of the patient’s personal information
- Demonstrate adaptability in being asked to complete a variety of tasks to help facilitate patient’s access to proper care.
- Contribute to the constant improvement of our healthcare process
- Establish positive rapport with patients, physicians, and other members of their healthcare team by instilling confidence, comfort, and dependability
Educational, Experience, and Knowledge Requirements:
- Registration as a Pharmacist with the College of Pharmacists of British Columbia.
- Knowledge of evidence-based pharmacotherapy and familiarity with tertiary resources including UpToDate, Lexicomp, RxFiles, and others.
- Proficiency in various computer programs such as Google Chrome, Word, Excel, and Adobe Acrobat
- Excellence in written and verbal communication is required. The majority of our patient interactions are conducted remotely.
- Fluency in English (including medical terminology) is required. Multilingualism is an asset
- Working Experience in BC HealthCare system is required (Clinical Rotations through Residency or School are sufficient)
- Experience with the following specialist disciplines are not required, but would be an asset: rhinology, respirology, immunology, neurology, and dermatology
Pay: $50.00 per hour
