Overview
We are seeking a dedicated and compassionate Case Worker to join our team. The ideal candidate will play a crucial role in providing support and guidance to individuals and families in need. This position involves assessing client needs, developing service plans, and connecting clients with appropriate resources. A background in social work or related fields, along with supervising experience, is highly desirable.
Responsibilities
- Conduct thorough assessments of clients' needs and circumstances to develop individualized service plans.
- Provide ongoing support and advocacy for clients, ensuring they have access to necessary resources and services.
- Collaborate with community organizations, service providers, and other stakeholders to facilitate client referrals.
- Monitor client progress and adjust service plans as needed to ensure effective outcomes.
- Maintain accurate and confidential case records in compliance with organizational policies.
- Participate in team meetings and case consultations to discuss client progress and share best practices.
- Provide supervision and guidance to interns or junior staff as needed.
Requirements
- Proven experience in case management or social services is essential.
- Supervising experience is a plus, demonstrating leadership abilities within a team setting.
- Strong interpersonal skills with the ability to build rapport with clients from diverse backgrounds.
- Excellent organizational skills and attention to detail for managing multiple cases effectively.
- Ability to work independently while also being an effective team player.
