Duties

- Administer medications, including injections and IV therapies, ensuring proper documentation and adherence to safety protocols
- Manage patient care through case management strategies, coordinating services for patients with complex health needs such as dementia or chronic conditions
- Perform procedures such as phlebotomy, laboratory specimen collection, and pain management interventions
- Utilize electronic health record systems like Epic and Cerner for accurate documentation of patient encounters and treatment plans
- Collaborate with multidisciplinary teams to ensure seamless patient care in settings like nursing homes, assisted living facilities, hospitals, or home care environments
Skills
- Proven experience working with children, seniors, and diverse patient populations in various healthcare settings
- Strong knowledge of medical terminology, medication administration, and clinical procedures
- Experience in nursing home environments, hospital units, or home health care is highly desirable
- Skilled in case management, dementia care, pain management, and acute care practices
- Proficiency with electronic health record systems such as Epic and Cerner for documentation and order entry
- Excellent patient care skills with a compassionate approach to supporting individuals through their health journeys
- Familiarity with laboratory procedures like phlebotomy and laboratory testing processes
- Ability to communicate effectively with patients, families, and healthcare teams while maintaining professionalism and confidentiality
