Davenport-Perth Neighbourhood and Community Health Centre (DPNCHC) is a multi-service organization that provides a wide variety of health and community programs and services for a diverse community. Our Health team is seeking an energetic community dietitian who will focus on families in the Davenport Community. You will provide a mix of counseling and assessment, assist the health and community teams with program planning and facilitate group health educational and support programs.
- Abbreviated Responsibilities:
- Assess, counsel, support and advocate for clients with a special focus on clients with food insecurity and mothers/caregivers and their children to improve their medical, nutritional and psychosocial status, and promote access to programs/services,
- Provide expertise around working to improve the health and nutrition of breastfeeding mothers and the successful transition for children to solid foods,
- Provide assessment and counseling to general client population and consultation services to other primary care and frontline workers involved with the client population,
- Lead and participate in program development, implementation and evaluation and make recommendations to the team,
- Collaborate with the community and other health care agencies to develop community-based programs (focused on chronic disease prevention and management, and food insecurity) utilizing a community development approach,
- Keep accurate statistics and records on all information and case contacts as required to be kept under the rules and regulations of the Centre and its duties and responsibilities to its funders.
Qualifications:
- Bachelor degree in human nutrition and dietetics with accredited practical training (i.e. integrated undergraduate degree, post-degree master’s, diploma practicum program, or postgraduate practicum program),
- Eligible for membership with the College of Dietitians of Ontario,
- 3-5 years experience and commitment to community-based healthcare and knowledge of community-based resources, health promotion, and community development,
- Demonstrated experience in community nutrition including effective counseling and group development skills,
- Thorough knowledge of and proficiency in nutrition care,
- Experience working with culturally diverse populations, especially knowledge of and sensitivity to the impact of social, economic, environmental and cultural issues in diet and nutrition,
- Demonstrated commitment to social change and cultural competency,
- Self-directed and independent but also proven experience in working effectively as an inter-disciplinary team member and in collaboration with other community professionals,
- Ability to work flexible hours, including evenings and weekends,
- Ability to speak Spanish and/or Portuguese, considered an asset,
- Knowledge of the Davenport Perth community an asset.
Salary Range: $71,656 - $83,550 per annum, plus Extended Health Benefits and Healthcare of Ontario Pension Plan (HOOPP). This position is part of the bargaining unit.
