Stonegate CHC has been providing primary health care, health promotion and a wide variety of community programs to the South Etobicoke Community since it was created by community members in 1992. Our Mission is to improve the well-being of our community. Purpose
Working within a collaborative and interdisciplinary model of health and wellbeing, the Nurse Practitioner is responsible for the provision of primary health care services in accordance with the College of Nurses of Ontario Standards of Practice for Registered Nurses in the Expanded Class and the Expanded Services for Patients Act. This role is vital in addressing population and individual health concerns, ensuring clients receive timely, high-quality care in a dynamic clinical environment. The NP position has a strong emphasis on health promotion and disease prevention and works within the social determinants of health framework supporting primary care clinics and initiatives. Current and valid registration in the Extended Class with the College of Nurses of Ontario is required.
Reporting
The Nurse Practitioner is a member of and accountable to the Clinical Team. The Nurse Practitioner reports to the Clinical Manager.
Reporting Responsibilities
- Provide primary health care services within the scope of the NP-PHC including: physical assessments, diagnosis and interpretation of test results, and development of treatment plans which includes health promotion/education/counselling; history taking, screening, referral, and follow up. Consult with physicians and/or other health care providers as needed.
- Supervise students by providing orientation, setting up schedules, developing/implement training/learning plans and conducting individual teaching with shared client contact.
- Participate in hiring, developing and maintaining clinical staff; participate in the supervision of clinical student placements, including graduate nursing, medical, NP-PHC, RN and others.
- Provide services during regular and extended hours on a scheduled basis and on urgent basis in a variety of settings such as: within the clinic, in community locations, satellite offices, home visits, hospital visits.
- Work within a health care framework which emphasizes health promotion and education, disease prevention, and recognizes the special risk factors (social, physical, psychological etc.) which may affect the health of clients.
- Work in partnership with other members of primary care to share responsibility for planning, provision and coordination of client care.
- Ensure appropriate, comprehensive treatment is delivered by maintaining complete and accurate medical records using the electronic client record, participating in chart reviews and discussions of problem cases.
- Perform preventive health measures and enter data compliant with the process for tracking MSAA accountability measures. Meet MSAA targets.
- Participate in administrative activities of the health centre by: providing input into program development, delivery and evaluation; collecting statistical data as required by the centre; identifying areas of development with regards to policies, procedure and protocols; participating in staff, team and other committees; liaising with other institutions, organizations and health and social service professionals.
- Maintain and develop professional competence through appropriate continuing education (i.e., peer interaction, literature review, conferences and courses).
- Provide after-hours on-call service on a rotational basis.
Key Qualifications
Required:
- Masters prepared RN (EC) mandatory
- Active registration and good standing with the College of Nurses of Ontario as a Primary Health Care Nurse Practitioner
- Availability to work a weekly Monday or Tuesday evening shift.
Preferred:
- Experience working in a primary health care setting, or a combination of hospital, community, and/or public health settings.
- Nursing experience working with priority populations within a multidisciplinary health care environment.
- Experience in episodic care, primary care, urgent care, or an equivalent clinical setting.
- Experience conducting comprehensive health assessments, including obtaining relevant medical and family histories, identifying pre-existing conditions, and reviewing current medications.
- Strong cultural competence and the ability to recognize, respect, and respond to diverse cultural backgrounds and perspectives.
- Experience in chronic disease management (e.g., diabetes and hypertension), including providing patient and family education and support.
- Demonstrated proficiency in current physical assessment and treatment practices within the Nurse Practitioner–Primary Health Care scope of practice.
- Demonstrated commitment to and knowledge of community-based health care.
- Ability to establish and maintain therapeutic relationships with a diverse client population.
- Proficiency in software applications related to primary health care, including electronic medical record systems (experience with TELUS PS Suite is considered an asset).
- Excellent interpersonal, written, and verbal communication skills, with strong problem-solving, organizational, and time-management abilities.
- Thorough knowledge of therapeutic approaches and evidence-informed practices grounded in health promotion and disease prevention principles.
- Ability to work collaboratively within an interdisciplinary health care team, including physicians, nurses, social workers, dietitians, physiotherapists, and community-based service providers.
Asset:
- Experience with trauma-informed care
- Experience in navigation to help clients access the resources they need to receive quality care
- Strong digital/computer literacy skills and comfort with digital/virtual platforms and service delivery
- Knowledge of issues affecting low income, multi-lingual, and racialized, and 2SLGBTQI+ communities
- Experience with working with low-income, multilingual, multiracial populations and communities
- Ability to speak multiple/second language(s)
