Role Summary
The Social Worker role at The Safehaven Project for Community Living (Safehaven) provides a range of clinical services, case management and advocacy, as well as community outreach, consultation and program implementation. This role is also responsible and accountable for the maintenance of strong standards of quality as far as satisfaction and utilization of respite beds, along with providing a range of clinical services, case management, and advocacy as well as community outreach, consultation and program implementation.
Responsibilities:
- Providing a range of clinical services to individuals, families and/or groups with an assigned caseload
- Providing assessments, case coordination, liaison, and service planning with community resources
- Participating in case reviews, program planning and evaluation
- Providing coverage to intake process including consultation to referring services/families
- Providing clinical supervision to students
- Raising awareness and educational presentations
- Maintaining documentation and records
- Participating in case conferences for client plans of care
- Participating as a member of the On-Call Team
- Other duties may assign to you by your supervisor and indirect supervisor according to the needs of the business/ organization
- You will also be required to work/ visit other locations within GTA as per operational needs.
Qualifications:
- Bachelor's of Social Work, preferably a Master of Social Work, must be registered with OCSWSSW
- Knowledge of the Developmental services sector;
- Minimum three (3) years of clinical experience or other relevant experience;
- Thorough knowledge of social work, including group work, community organization, consultation, education, and research;
- Thorough knowledge of the scope and activities of local and private health and welfare agencies and community organizations;
- Advanced written and verbal communication skills;
- Demonstrated advanced computer proficiency and knowledge of social media;
- Must have a valid driver’s license and access to a reliable vehicle;
