As a key member of the health care team, you will monitor and deliver nursing care at homes/ community homes, in accordance with federal and provincial legislated standards of practice. You will utilize your sound knowledge, skills and judgment to offer safe, compassionate, and ethical nursing care in a community/in home setting.
The individuals supported in these communities are female and require assistance with personal care, daily living activities, and community participation in a manner that prioritizes privacy, dignity, comfort, and trust within the support relationship.
This requirement is based on the nature of the care environment and the needs of the individuals served and is considered a bona fide occupational requirement under applicable human rights legislation.
Duties and Responsibilities
- Following the care plan, observing and reporting any substantial findings and/or changes in patient/resident/client’s behaviour to the appropriate registered member of the healthcare team.
- Working under the supervision of a Registered Healthcare Professional such as a Registered Nurse (RN) or a Registered Practical Nurse (RPN)
- Perform tasks delegated to them (only if they are trained to perform the delegated task)
- Assisting with ambulation, positioning, and transferring using mechanical lifts such as a Hoyer lift.
- Assisting or providing total personal care such as toileting, bathing, and perineal care.
- Assisting with eating, dressing, and grooming.
- Performing sitter duty or patient watch for confused patients in clinical care settings.
- Computer or paper documentation of Activities of Daily Living (ADL’s) and other findings.
- Reporting behavioural and clinical changes to a Registered Nurse, Registered Practical Nurse, Resource Nurse, or Manager.
- Self-actualization by helping patient/resident/client to reach maximum potential.
Qualifications
- Personal support worker certificate from a designated institution.
- Must possess strong written and oral communication, problem solving, decision making skills, and time management skills.
- Knowledgeable in computer charting and electronic medication administration programs.
- Current Basic Cardiac Life Support and First Aid certification.
- Current Vulnerable Sector Screening.
- Complete Immunization Record.
- Valid driver’s license and reliable vehicle.
- Community experience is an asset!
