Hospice Fredericton is looking for LPN's to fill both permanent and casual positions to be a part of the delivery of care to Hospice patients and families.
Duties & Responsibilities:
· Report to the Charge Nurse as part of the care team the LPN assists with the organization and delivery of end-of-life care.
· Practice at full scope in a manner consistent with professional, ethical, and legal standards of practice for LPNS.
· Assess, plan, evaluate and implement patient centered care.
· Administers medications and provides treatments according to Physician orders following all medication management policies, guidelines, and scope of practice.
· Responsible for communicating patient status/needs to Physicians and providing symptom control to patients based on patient centered assessments.
· Assist in development of patient directed care plans involving all members of the interdisciplinary team.
· Maintain standards of accurate and concise documentation and reporting.
· Provide direct patient care and assist with ADLs such as bathing, dressing, skin care, and mobilizing when necessary.
· Provide ongoing teaching, education and support to patients/families in a caring non-judgmental manner.
· Delegate tasks and duties to other members of care team in a caring professional manner.
· Assumes a leadership role and provides leadership to care team and oversee the provision of quality health services, respecting the patient's right to individualized planning of end-of-life care.
· Facilitate teamwork among the care team and foster a positive work environment.
· Act as an advocate on behalf of patients, families, and co-workers.
· With support from the organization maintain an optimal level of wellbeing and balance and display behavior consistent with adherence to organization standards
Skills and Qualifications:
The LPN is a responsible professional with current registration and experience in end-of-life care.
· LPN license in good standing
· Caring & Compassionate
· Team player
· Organized, takes initiative.
· Ability to work with a wide range of individuals.
· Strong interpersonal, organizational, and communication skills.
· Use sound judgment, diplomacy, and confidentiality.
· Fluent in English, spoken French an asset
· Experience in Palliative Care with be an asset
· Must have a clear Vulnerable Sector Check
· Valid CPR/BLS certification
