Position: Case Manager - Addiction
Status: Casual
Program: Addiction Services
Initially assigned to: Oak Street
Purpose:
Under the direct supervision of the Program Manager, the Case Manager – Addictions will provide individualized support and guidance to residents throughout the treatment process. They will assess residents’ progress through the program, monitor recovery, and identify relapse risks. The Case Manager will collaborate with residents to develop recovery plans, set achievable goals, and provide guidance and support throughout the process. Additionally, the Case Manager will facilitate psychoeducational and therapeutic group sessions with residents aimed at enhancing resident goals and providing tools to support their long-term recovery
Education and training:
· Proof of successful completion of a post-secondary education in Social Service Work, Addictions Counselling or Nursing and proof of registration with the College of Nurses (if applicable)
Requirements:
· A negative Police Vulnerable Sector Check – Level 4 Clearance
· Correctional Services Canada (CSC) Clearance or willingness to obtain
· A valid class G driver’s license and proof of insurance
· Daily access to a reliable vehicle
· Current First Aid and CPR certificate required
· Current Non-Violent Crisis Intervention (NVCI) training or willingness to obtain
· Current Applied Suicide Intervention Skills Training (ASIST) or willingness to obtain
· Current Safe Food Handling course or willingness to obtain
· Able to lift at a minimum 20 lbs. from squatting to standing position
· Bilingualism (English/French) is an asset
Experience:
· Minimum of 2 years of experience in addiction & mental health services
· Minimum 1 to 3 years of experience working with vulnerable populations, specifically adults living with a serious and persistent mental illness and/or acquired brain injury (ABI) and/or Addictions and/or complex healthcare needs providing assessments and rehabilitation services
· Experience working in a community setting and working knowledge of community resources
Knowledge/Skills/Abilities:
· Basic working knowledge of computer software
· Ability to work cooperatively and independently with minimal supervision and as part of a team
· Effective problem solving and decision-making skills
· Proficiency in managing challenging behaviours
· Excellent interpersonal/verbal/written communication skills
· Ability to maintain confidentiality as per PHIPA (Personal Health Information Protection Act)
· Working knowledge of the Mental Health Act, with a working knowledge of Bill 68 inclusive of Community Treatment Orders, Residential Tenancy Act and the Health Care Consent Act
· Interpersonal skills in establishing rapport with and counselling clients, and in relating to other professionals
· Analytical skills to assess client needs; ensure appropriate referral and case management, and to prepare written reports
· Comprehensive knowledge in addictions and mental health including awareness of current research, issues and trends as they relate to therapeutic approaches
· Knowledge of community, provincial and regional organizations, and resources
· Knowledge/experience using the principles and practices of psychosocial rehabilitation
