We are currently seeking a Full Time LPN Clinical Systems Coordinator to join our team.
Position Overview
This full time management position offers an excellent work-life balance with a regular Monday to Friday schedule and flexible working hours. As a non-shift role, you will have the opportunity to collaborate closely with interdisciplinary teams, influence best practices, and contribute to the ongoing success of our clinical programs.
This is an exciting opportunity for a motivated and detail-oriented LPN to take on a key leadership role within our organization. As the Clinical Systems Coordinator you will play an integral part in supporting clinical excellence, regulatory compliance, quality improvement initiatives, and exceptional resident-centered care across our community.

Key Responsibilities
- Coordinate resident care through accurate and timely completion of MDS assessments.
- Develop and maintain individualized resident care plans.
- Participate in and facilitate resident care conferences.
- Complete and manage ACCIS, DLIP, AUA, and ALA reporting requirements.
- Conduct unit audits and support ongoing quality improvement initiatives.
- Review and ensure compliance with CCHSS and Accommodation Standards.
- Collaborate with the Care Management Team to provide clinical support across all care units.
- Maintain audit readiness and support regulatory compliance activities.
- Participate in a rotating on call schedule.
Qualifications
The ideal candidate will possess:
- An active CLHA Licensed Practical Nurse (LPN) license.
- Strong computer proficiency and experience with clinical documentation systems.
- Excellent organizational skills and attention to detail.
- The ability to manage multiple priorities while maintaining accurate and timely documentation.
- Strong interdisciplinary collaboration and communication skills.
- A commitment to providing high quality resident centered care.
- Experience with audits, regulatory standards, and quality assurance processes is considered an asset.
Employment Details
Position: Full-Time
Hours: 40 hours per week
Schedule: Monday to Friday, with flexible working hours
