The Dr. Peter Centre provides compassionate HIV care for people living with significant health issues including mental illness, trauma, substance use, unstable housing and poverty.
Through its day health, 24-hour care residence and enhanced supportive housing programs, the Dr. Peter Centre provides healthy meals, counselling, therapies, nursing, and a safe place for peer socialization and support. This integrated model of care successfully engages individuals in their health care, improving adherence to HIV treatment and overall health.
We are currently looking for a Social Worker to join our team on a temporary full-time basis.
Position Summary
The social worker is a member of the multidisciplinary team at the Dr. Peter Centre, and provides case management to the Enhanced Supportive Housing (ESH) participants and counseling support to the residence and the ESH programs. Additionally, this role will assist with forms and processes that Day Health participants may need help with. The position will require strong supportive counseling skills, ability to case manage clients, and expertise in the areas of trauma informed care, mental health, substance use and palliation.
Key Responsibilities:
The Social Worker is responsible for the following:
- Providing case management and supportive counseling to the ESH participants and residents residing in the residence;
- Liaising with appropriate referrals with regard to medico-legal issues such as financial support, power of attorney; court appearances and probation; competency assessments.
- Facilitating admissions, transfers and discharges within residential, enhanced supported housing programs and transfers into the day health program. This includes orientation to services and liaison with community providers;
- Supporting family/significant others; grief and loss counseling;
- Documenting significant interventions and contacts in electronic clinical record;
- Liaising with appropriate referrals with regard to medico-legal issues such as creating wills; establishing power of attorney; court appearances and probation; competency assessments;
- Assisting Day Health Participants with community referrals and forms as needed;
- Developing and facilitating health and wellness groups for residents;
- Documenting significant interventions and contacts; and
- Other related duties including assignment to day health program.
Education, Qualifications and Experience:
The preferred education, qualifications and experience of the Social Worker are:
- Masters’ Degree Social Work or related field preferred.
- Two years of related experience (HIV, Substance Use, Mental Health).
- Experience working with people who are living with HIV/AID.
- Experience in Trauma informed care, Conflict Resolution and Restorative Practice.
- The ability to use sound clinical judgment in a variety of circumstances.
- The ability to be self- directed.
- The ability to adapt clinical practices to meet the needs of the participants.
- The ability to provide counseling services within an interdisciplinary team.
General Working Conditions
This is a temporary full-time position (Monday to Friday 35 hours per week) with an hourly pay rate of $42.27-$52.81 per hour. This position requires flexibility and the ability to prioritize workload, and the ability to work in a fast-paced environment where demands and deadlines may change with short notice. Multitasking and flexibility are a must in order to meet deadlines, and to support many operational functions/activities that may occur concurrently or without notice. An understanding of the importance of maintaining the confidentiality of sensitive employee/client information is necessary.
