Position Summary
We are currently seeking a qualified Behaviour Support Worker in the Penetanguishene area, to work under the direction of a Behaviour Therapist and work closely with our Clinical Director/Psychologist as part of a team supporting clients who are experiencing behavioural and psychological symptoms resulting from cognitive impairment, dementia and/or acquired brain injury (ABI).The Behaviour Support Worker along with clinical leaders will complete client observations and assessments, collaborate on developing appropriate interventions, model strategies and assist with transitions across sectors.
Duties & Responsibilities

Provide transitional care support e.g. from hospital to a transitional care unit (TCU), supportive living residence, or home.

Model appropriate behavior, provide positive reinforcement and corrective feedback.

Assist with Activities of Daily Living (ADLs). This includes bathing, transfers, toileting, and physical assistance; as well as assisting with participation in physical, vocational, and recreational activities. Light housekeeping as needed.

Assist with connection to community supports for specific populations as required (e.g. diabetes care, recreational programs, addictions support, housing and food insecurity, etc.)

Assist with and implement behavioural and rehabilitation interventions and plans as prescribed by the multidisciplinary team, to ensure quality supports.

Collect data on client behaviour through direct observation, interviews with client’s circle of care, and other standardized assessments.

Assist with administration of medication programs.

Complete documentation/reports following company and provincial guidelines.

Report significant clinical findings to the Clinical Director as appropriate.

Participate in team meetings as required.

Provide educational support to staff and caregivers regarding behavioural care plans.

Participate in team and/or organization-wide committees as requested.
-
Participate in the evaluation of risks associated with maladaptive behaviours upon client intakes and contribute to plans to mitigate the identified risks.

Maintain confidentiality of client and corporate information and discuss same only with appropriate Bayshore personnel.

Ensure the protection of client rights; carry-out an advocacy role.

Remain current on clinical best-practices and evidence-based interventions.

Participate in ongoing internal and/or external continuing education activities.

Adhere to Bayshore Policies and Procedures.

Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.

Participate in proactive Health & Safety activities while performing all duties; responsible to notify immediate Supervisor of any Health & Safety risks or concerns.

Qualifications:
Education / Licenses / Certification
-
Post-secondary education in behaviour sciences, mental health/addictions, psychology, and/or related areas.
-
Standard First-Aid + CPR-C.

Minimum G2 driver’s license
Experience
-
Minimum 2 years assisting with behaviour assessments and collaborating on behaviour interventions.
-
Experience working with individuals with various types of dementia, delirium, cognitive issues, and complex behavioural challenges.
-
Knowledge of psychogeriatric assessment tools and behavioural, cognitive, psychosocial, and environmental assessments relating to responsive behaviours.
-
Practical experience supporting individuals in an acute care and/or mental health care settings; application of appropriate crisis, de-escalation, and behavioural management strategies.
-
Demonstrated experience working with dual diagnosis is considered an asset.
-
Personal Support Worker (PSW) experience is considered an asset.
-
Transitional care support experience is considered an asset.

Availability to work on a casual basis with flexibility for weekend, day, or evenings shifts.
