Overview
We are seeking a compassionate and dedicated Counsellor / Social Worker to join our healthcare team. This role involves providing emotional support, case management, and patient-centered care to clients with substance abuse within a rehabilitation clinical setting. The ideal candidate will have experience working with adults , familiarity with terminology, and a strong commitment to improving patient outcomes through comprehensive support services.
Duties
- Conduct individual and group counseling sessions for patients addressing emotional, psychological, and social needs.
- Collaborate with medical teams to develop and implement care plans tailored to each patient's unique circumstances.
- Assist with case management by coordinating services, resources, and referrals for patients requiring additional support.
- Collect and analyze data related to patient progress, treatment effectiveness, and service delivery for continuous improvement.
- Provide education on health conditions, treatment options, and coping strategies to patients and possibly their families.
- Support patients in community resources as needed.
- Maintain accurate documentation of patient interactions, assessments, and care plans in compliance with healthcare regulations.
- Engage in research activities related to patient care to enhance service quality and evidence-based practices.
Qualifications
- Proven experience working with patients in a healthcare or social services environment.
- Knowledge of medical terminology, and patient care protocols.
- Strong case management skills with the ability to coordinate multiple services effectively.
- Experience with data collection, analysis, and reporting related to patient outcomes or program evaluation.
- Excellent communication skills for working with co workers, medical staff, and educational professionals.
- Ability to handle sensitive situations with empathy, professionalism, and confidentiality.
- Relevant qualifications in social work, counseling, psychology, or related fields are preferred.
