We are launching a new, outcome-driven program focused on creating lasting change in the lives of those we serve. As a Case Manager, you will join a collaborative and forward-thinking team dedicated to making meaningful, measurable impact for participants across Southern Alberta.
Position Summary
The Case Manager is responsible for supporting participants through the execution of individualized service plans. This role focuses on coordinating services, maintaining participant engagement, and ensuring alignment between therapeutic interventions and program requirements.
The Case Manager acts as the central point of contact for participants, providing structured support, accountability, and coordination between clinical services and external employment programming.
The ideal candidate is highly organized, compassionate, and experienced in working with individuals experiencing complex challenges such as mental health concerns, addiction recovery, and employment instability.
Key Responsibilities
- Support participants in following individualized service plans
- Provide ongoing check-ins to ensure participant engagement and progress
- Reinforce strategies and goals identified in therapy
Case Management & Support
- Provide ongoing one-on-one support to participants throughout the program
- Monitor client progress and adjust service plans as needed
- Support clients in addressing barriers related to mental health, addiction, housing, and employment readiness
Schedule and coordinate participant involvement in:
- Therapy sessions
- Group programming
- Employment services
- Act as liaison between participants and service providers
Progress Monitoring & Accountability
- Track attendance, participation, and engagement
- Identify barriers to participation and escalate concerns to clinical or program leadership
- Support participants in maintaining consistency and accountability
Documentation & Reporting
- Maintain accurate, timely, and confidential records of participant interactions
- Track service delivery metrics in accordance with program requirements
- Contribute to reporting requirements, including monthly and interim reports
Follow-Up & Retention
- Conduct follow-up with participants post-program to support sustained outcomes
- Track long-term success indicators and engagement
Qualifications
- Minimum Diploma in Social Work, Psychology, Human Services, or a related field
- Minimum 2–3 years of experience in case management or a related role
- Experience working with individuals facing complex barriers (mental health, addiction, unemployment)
- Strong understanding of trauma-informed and client-centered practices
- Excellent organizational, communication, and interpersonal skills
- Ability to work independently and as part of a multidisciplinary team
