Niagara Medical Group Family Health Team is an interdisciplinary primary health care team in Niagara Falls, On. Located on Queen Street, just down from City Hall. We are seeking a dedicated Nurse Practitioner to join our team. The ideal candidate will have a passion for working with diverse populations and providing clinical and therapeutic services.
Position Requirements:
Perform patient health assessment and health maintenance activities
Monitor the ongoing therapy of patients with mental health illness
Diagnose and treat acute/episodic minor illnesses
Offer phone and home consultation/care for acute and episodic illnesses
Consistently record and document in the patient’s electronic medical record
Determine the need for and order all necessary diagnostic imaging, medications, treatments and referrals
Prepare and report quarterly data for Ontario Health
Education: Completion of college/university/CEGEP/vocational or technical training
Credentials (certificates, licenses, memberships, courses, etc.):
Nurse Practitioner
Current Registration in good standing with the College of Nurses of Ontario as a Registered Nurse, Extended Class with Nurse Practitioner (Adult)
Master’s degree in nursing and/or equivalent graduate Degree
Miscellaneous
Basic computer knowledge and competence (word processing/clinical applications)
Electronic Medical Record knowledge is an asset
CPR/Basic First Aid Certification
Experience:
One to two years’ clinical experience as a Nurse Practitioner
Clinical experience in Mental Health an asset
Job Types: Full-time, Permanent
Pay: $120,000.00-$125,477.00 per year
