Job Summary
We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our dynamic healthcare team. Responsibilities
- Perform thorough patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
- Administer medications and injections accurately following established protocols, including IV infusions and other advanced procedures.
- Manage complex cases such as dementia care, Alzheimer's care, memory care, and patients with developmental disabilities.
- Utilize electronic health record (EHR) systems such as Epic or Cerner to document medical records, treatment plans, and patient progress efficiently.
- Provide specialized care in areas like hospice, palliative care, acute pain management, and infectious disease treatment.
- Assist with diagnostic evaluations such as specimen collection/processing, medical imaging coordination, and catheterization procedures.
- Collaborate with multidisciplinary teams to develop discharge planning strategies and coordinate post-acute or home & community care services.
- Support emergency situations with airway management, ventilator support, or triage assessments in high-pressure environments like Level I Trauma Centers or ICU settings.
- Educate patients and families on health coaching topics including medication administration, nutrition, chronic disease management, and preventive care.
Qualifications
- Valid registration as a Practical Nurse (RPN) with the applicable licensing body.
- Proven experience in diverse healthcare environments such as hospital medicine, ICU, emergency room, or long-term care facilities.
- Strong knowledge of medical terminology, anatomy & physiology, ICD coding (ICD-9/10), CPT coding, and medical documentation standards.
- Hands-on experience with EMR/EHR systems like Epic or Athenahealth is highly preferred.
- Demonstrated skills in IV insertion/insertion techniques and sterile processing procedures.
- Experience working with vulnerable populations including pediatrics (neonatal ventilator), geriatrics (dementia & memory care), or individuals with disabilities.
- Ability to perform physical examinations and assessments accurately while maintaining patient safety at all times.
