Job Overview
The Tri-County Women's Centre (TCWC) is seeking an innovative, collaborative, and community-minded Community Health & Wellness Outreach Lead to help lead the implementation of our new Primary Health Care Prevention Initiative. This exciting position will strengthen community-based primary health care by coordinating preventative health and wellness services, community outreach, health promotion, and partnerships throughout Yarmouth, Digby, and Shelburne Counties.
Responsibilities
- Coordinate the planning and logistics of community outreach initiatives by booking and managing outreach spaces for wellness clinics, health care services, educational workshops, support groups, and health promotion activities across Yarmouth, Digby, and Shelburne Counties.
- Build and maintain collaborative relationships with community partners, organizations and government agencies to expand access to services and improve community-based care.
- Provide community health & wellness navigation across the tri-counties, advocacy, and some basic client support.
- Connect individuals to the TCWC Wellness Clinic, other healthcare providers, TCWC services and supports, community resources, and social supports.
- Complete Intakes and assign to the appropriate service provider or program with TCWC and the TCWC Wellness Clinic and community.
- Coordinate and deliver health promotion, wellness, and prevention initiatives.
- Build partnerships with providers, community organizations, and government agencies.
- Support the ongoing initiatives of Tri-County Women’s Centre and the TCWC Wellness Clinic.
- Plan and facilitate community outreach, education sessions, and wellness programming.
- Collect program data, monitor outcomes, and contribute to reporting and evaluation.
- Travel throughout Yarmouth, Digby, and Shelburne Counties to support clients and community initiatives.
Requirements
- 3-4+ years experience
- Degree or Diploma in relevant fields such as Nursing, Social Work, Health Promotion, Community Health, Public Health, Health Sciences, Community Development, Health Administration, and/or Other relevant combination of education and experience may be considered.
- Experience working with vulnerable and underserved populations.
- Strong working knowledge of Electronic Medical Records (EMR), with experience using TELUS Health EMR considered a significant asset.
- Experience in program coordination and community engagement.
- Ability to accurately document client interactions while maintaining confidentiality and privacy standards.
- Where applicable the successful candidate must hold all required licences, certifications, and current registration in good standing with the applicable regulatory college, licensing authority, or professional association relevant to their profession.
- A clear understanding of confidentiality and the dynamics of a “supportive” relationship, with the ability to maintain confidentiality within these parameters
- Bilingualism is an asset
- Successful completion of Criminal Record Check - Vulnerable Sector Screening & Child Abuse Registry Screening is required.
