This is a temporary position and the expected duration for the role would be approximately 12 - 18 months.
This innovative facility integrates data and technology into patient assessment, care delivery, and communication, while providing specialized, person-centred support for individuals who no longer require hospital-level treatment but need additional care to safely return home or transition back into their community. Our team is committed to dignity, safety, and clinical excellence in every interaction.
Reporting to the Manager of Transitional Services, the RN is integral in supporting the goal of returning to home/community by being engaged in interprofessional collaboration, offering evidence based clinical expertise to inform care plans that fit with patient/family goals, using effective communication strategies, shared decision-making, and mutual support.

About the Opportunity
Assesses, plans, implements, directs, and evaluates nursing care and functions, in accordance with the patient-directed approach to care through the established philosophy of service.
Observes, records and reports symptoms, reactions, treatments, and changes in the patient’s condition.
Documents according to facility policy e.g Daily Patient Care Record, Medication Administration Record (MAR), patient progress notes, initiating medication incident reports, incident reports, and other facility specific documentation as required.
