Position Summary
The Personal Support Worker (PSW) provides compassionate, safe, and client-centred care across both the Transitional Care Unit and community home care settings. This position is designed as a blended role, supporting client care needs in both environments based on operational requirements.
The successful candidate must be comfortable and available to provide care in both the TCU and community settings as part of this position.
Position Schedule and Availability
This position includes responsibilities in both the Transitional Care Unit and community home care.
- Scheduled shifts within the TCU will include four (4) shifts per bi-weekly pay period.
- The role also requires availability to support approximately 10 hours per week of community home visits, based on client needs, scheduling requirements, and operational demands.
- Scheduling will be coordinated based on client assignments, staffing needs, and service requirements.
As this is a combined TCU and community position, the ability to support both areas of service is an essential requirement of the role. Employees must be available to fulfill the responsibilities associated with both the TCU and community components of the position.
Key Responsibilities
- Provide personal care and support to clients in accordance with individualized care plans and CareKW standards.
- Assist clients with activities of daily living, including personal hygiene, mobility, toileting, dressing, and nutritional support.
- Promote client dignity, independence, safety, and overall well-being.
- Provide care in both TCU and community environments as assigned.
- Complete accurate and timely documentation of care provided.
- Observe and report changes in client condition and care needs.
- Follow infection prevention and control practices, workplace safety requirements, and organizational policies.
- Communicate effectively with clients, families, and healthcare team members
