Job Overview
We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our Primary Care team. The ideal candidate will provide high-quality patient care in a fast-paced clinical environment, supporting patients across various age groups and health conditions. This role offers an opportunity to work in a dynamic healthcare setting, utilizing your nursing expertise to improve patient outcomes and promote wellness. Experience in nursing homes, hospitals, acute care, dementia care, pediatrics, home care, or senior care is highly valued. The RPN will be responsible for delivering comprehensive nursing services, including medication administration, patient assessments, and care planning, while working with advanced electronic health record systems such as Epic and Cerner.
Responsibilities
- Receive client calls and visits that are primary care related
- Have RPN status and current registration in the Collegeof Nurses of Ontario
- · Experience in reception, appointment scheduling, triage assessment
- Possess excellent problem-solving, communication and interpersonal skills
- Possess computer literacy and have experience with Electronic Medical Records
- Second language a valuable asset
- Provide triage at reception, referring client to appropriate level of care
- Schedule client appointments, provide client follow-up calls
- Provide direct and indirect client care services (e.g., injections, dressing changes, assist with medical procedures)
- Utilize Electronic Medical Records system for documentation and information retrieval
Qualifications
- College diploma and two to five years of experience in a health care setting accomplishing similar duties.
- Current certification with the College of Nurses of Ontario
- Demonstrated ability to be flexible and adaptable
- Possess excellent assessment, organizational and problem-solving skills.
- Excellent communication and interpersonal skills
- Enthusiasm to serve a diverse community
- Proficiency in another language an asset
- Proficiency in the use of personal computers and software
- Completion of CPR (cardiopulmonary resuscitation) training and first aid courses.

Pay: $50,869.00-$59,846.00 per year
