Job Overview
We are seeking a dedicated and compassionate Registered Practical Nurse (RPN) to join our healthcare team and serve as the clinical backbone of the patient experience. The ideal candidate will possess a strong foundation in nursing practices and the ability to provide high-quality, personalized care to diverse patient populations, including children, seniors, and individuals requiring specialized support, such as dementia or hospice care.
Drawing on a versatile nursing background—with experience that may span across hospitals, nursing homes, assisted living facilities, or home care environments—you will play a vital role in delivering safe, effective care that promotes patient well-being and recovery. You will utilize your excellent communication skills to work seamlessly with patients, families, and physicians within our fast-paced environment.
Responsibilities
- Administer medications and treatments accurately following physician orders and established protocols
- Monitor patient health status through assessments and vital signs.
- Assist with activities of daily living (ADLs), dressing, mobility assistance, and nutritional support
- Perform phlebotomy and laboratory specimen collection as needed
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Educate patients and their families about health conditions, medication management, and post-discharge care instructions
- Ensure compliance with infection control standards and safety procedures in all patient interactions
- Participate in case management activities to coordinate ongoing patient care and resources
- Maintain accurate documentation of patient progress, medication administration, and incident reports
Requirements
- Valid registration as a Registered Practical Nurse (RPN) with the appropriate licensing body
- Proven experience in nursing home settings, hospitals, acute care, or home care environments is highly desirable
- Knowledge of medical terminology, medication administration protocols, pain management strategies, and case management principles.
- Experience working with diverse populations including seniors requiring senior care or hospice services; pediatric experience is a plus
- Strong communication skills to effectively interact with patients, families, and healthcare teams
- Ability to work independently as well as part of a multidisciplinary team.
