Bridges Community Health Centre (CHC) is a non-profit, community-governed organization funded by the Province of Ontario. Our holistic, interdisciplinary model of health and wellbeing focuses on primary health care, health promotion, illness prevention, and community capacity building, while considering underlying social determinants of health and barriers to accessing care. We are currently recruiting a full-time Registered Nurse (RN) to join our team.
The Registered Nurse (RN) functions interdependently with other members of the health care team within a health promotion framework (as defined by the World Health Organization). The RN provides confident and compassionate patient care, working to their full scope of practice, while also providing community outreach and education, individual health teaching and certain administrative tasks.
KEY RESPONSIBILITIES:
- Working to the full scope of practice for an RN as per College of Nurses of Ontario guidelines
- Processing new intakes; history taking and assessments;
- Providing clinical support to physicians and nurse practitioners as required;
- Health education and counseling of patients;
- Assessment and detailed triage of drop-ins and phone calls;
- Care of minor episodic illness within the context of established medical directives;
- Monitoring and education of patients with stabilized chronic diseases and long-term illnesses;
- Well child & Well women examinations;
- Performing specific procedures according to the College of Nurses of Ontario standards of nursing practice including lab tests, injections, suture removals, PAP tests, home visits, etc;
- Ordering medical supplies and maintaining clinical inventory as needed;
- Participating with other staff and community partners in programs both inside and outside of the health centre, providing education and group or individual counselling and follow-up;
- Acts as a health advocate on behalf of the patient and the community;
- Able to work independently without direction or supervision from other primary care providers;
- Supervises nursing students;
- Other duties as assigned from time to time.
QUALIFICATIONS INCLUDE:
· Registered and in good standing in the College of Nurses of Ontario (CNO)
· Current professional liability insurance
· Baccalaureate degree preferred
· Minimum three years’ experience as an RN in a community setting (primary care office experience an asset)
· Highly proficient in Electronic Medical Records and use of virtual tools
· Ability to practice utilizing a client-centered approach with a strong understanding of the impact of social determinants of health
· Excellent interpersonal skills and ability to positively collaborate with clients in a culturally diverse practice population, and with colleagues.
· Demonstrated ability to work effectively and efficiently in a fast-paced, interdisciplinary team environment.
· Excellent communication, problem-solving, conflict management and time management skills.
· Commitment to continual learning and professional growth
· Ability to work at either Bridges CHC location
ADDITIONAL REQUIREMENTS:
· Ability to work days, evenings and weekends as needed
· Visiting clients in various community settings
· Valid Ontario Driver’s License and access to a reliable vehicle
· Police Reference Check required
