Requirements:
- Certification in Family Medicine (CCFP) from the College of Family Physicians of Canada (CFPC)
- Current registration with College of Physicians of BC (CPSBC)
- At least one year of clinical, primary care experience
- Experience in a collaborative, team-based primary care setting and in chronic disease management preferred
