Primary Care Alberta is advancing a comprehensive, patient centred approach that strengthens safe transitions from hospital to community. The Integrated Care Program supports patients at high risk of readmission who require enhanced discharge planning and structured post discharge support. The program focuses on individuals with medical and psychosocial complexity, ensuring coordinated transitions from acute care to primary care and essential community supports. This role is ideal for nurses who are passionate about improving transitions in care and want to make a measurable impact on patient outcomes, health system integration, and continuity of care across sectors. As an Integrated Care Lead Nurse, you work at the intersection of acute care, primary care, and community services, bridging gaps and improving continuity of care. The ICL Nurse builds collaborative relationships with patients, families, and interdisciplinary teams to proactively support discharge planning. The ICL nurse helps clarify discharge goals, identify barriers, leverage existing resources, and facilitate referrals through appropriate discharge processes. Through comprehensive assessment, the ICL Nurse supports the development of individualized discharge plans and provides post discharge support to help patients navigate the health system and access primary care and community resources. By enhancing post discharge care, the ICL Nurse helps ensure patients remain supported within their primary care medical home, reducing avoidable emergency department visits and hospital readmissions. Beyond direct patient care, the ICL Nurse contributes to the growth of the Integrated Care Model by collaborating with colleagues across sites, sharing best practices, and supporting continuous quality improvement within Primary Care Alberta. As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of PCA. PLEASE NOTE: Provisional Permit holders will be paid $40.76 to $55.79 per hour. Upon obtaining a full practice permit, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article 25.02(b)(i) or (ii), as applicable.

Required Qualifications:
Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP). If you have not practiced as a Registered Nurse within the last five (5) years, completion of the following free courses is required: CRNA Medication Management - Chapters 1 - 4 (https://connect.nurses.ab.ca/home/%2Flearning-and-development%2Flearning-modules%2F) and the NextGenU Health Assessment Resources Certification (https://courses.nextgenu.org/course/view.php?id=271#section-0)
