Job Summary
We are seeking a dedicated and energetic Restorative Care Aide / Program Facilitator to support residents' well-being through personalized care and engaging activities. The ideal candidate will possess a strong background in senior care, with experience in restorative practices and working with individuals with disabilities. This role involves fostering independence, promoting health, and facilitating meaningful participation in daily activities within a supportive environment.
Duties
- To work in collaboration with nursing, management, and physiotherapy to assess, design, implement and evaluate complimentary individual nursing restorative care programs for residents who have been deemed as a nursing rehabilitation candidates.
- To document all data accordingly in the proper documentation in a timely manner and meet with the restorative team lead/management at a minimum once per month to discuss progress of the programs.
- To provide teaching/training to the nursing staff on the restorative care programs that are designed for the designated residents towards the end of their program and upon discharge.
- To evaluate, continue, discontinue individualized programs for residents as goals are met or no longer valid; on a quarterly timeline, ensuring sufficient documentation is provided to support the decision to continue tor cease the program under the direction of management/restorative care lead. Written report of resident’s and individualized nursing restorative program to are completed and handed in monthly to management/restorative lead.
- To work in collaboration with the Physiotherapist and PTA to assist with physiotherapy program as required.
- To collaborate with fall prevention team lead and management team to review residents with increase falls and assist with falls prevention programs as directed.
- To assist with completing Physiotherapy assessment referral requisitions and provide timely follow up to the wheelchair and walker requirements of residents, assist with vendor and PT visits and complete documentation post visits.
- To complete aids of daily living on ‘Resident’s Plan of Care’ to ensure that the information is up to dated to reflect the current status of the required aids of daily living is currently utilized by each individual resident. Total building assessment and logging of aids of daily living for all residents will be required as indicated by management.
- Responsible for day-to-day for the preparation, implementation and evaluation of recreation programs that meet a diverse population of residents.
- Ensures that every resident is involved in identified recreational activities in order to meet the goals of the resident's recreational treatment plan. Leads, instructs, and motivates residents in individual and group leisure pursuits.
- Setup and organize equipment needed for specific program(s), and oversee residents involved by encouraging participation.
- Ability to follow safety policy and protocol in the community with residents and volunteers.
- Effective and professional communication skills with all facility departments.
- Record and discuss residents' participation levels in programs.
- Escort, transport, assist residents to and from programs.
- Other duties as assigned.
Requirements
- Recreation and leisure services and / or SSW diploma.
- Physiotherapy assistant or athletic therapy education, or equivalent education from an approved college or university.
