Job Purpose:
The Personal Care Worker is a frontline role responsible for delivering high-quality, person-centered support to tenants within a 24/7 supportive housing environment.
This role combines direct care, mental health and substance use support, and operational leadership to ensure safe, consistent, and trauma-informed service delivery aligned with a Housing First and harm reduction framework.

Following prescribed policies and procedures associated with the housing program.

Supporting participants through a person-centered approach
The Personal Care Worker (PCW) will work with an interdisciplinary team to support participants of Brunswick Street - a supported housing program for senior to age in place with wrap-around supports. The interdisciplinary team provides 24/7 support to tenants around life skills and health and wellness to ensure participants can effectively manage their tenancy and lives.
Education and Experience:
Minimum 2+ years
- experience in:

Mental health and substance use (mandatory)
-
Harm reduction (mandatory)

Emergency shelter and/or transitional housing settings (mandatory)

Demonstrated ability to apply a trauma-informed approach
Assets:

Formal education in social, health, addictions, human services or community work

Lived/living experience related to the population served

Cultural understanding and relevant training

Diploma is an asset but not a requirement

Lived and/or living experiences of homelessness, mental health and substance misuse an asset
Demonstrated experience working with individuals with complex health and social needs-
including substance use, chronic illness, cognitive decline, and social isolation, homelessness and/or barriers to stable housing
