Duties
- Assist clients with daily living activities such as personal hygiene, meal preparation, and household tasks
- Support clients in accessing community resources and services
- implement individualized care plans
- Provide emotional support and encouragement to promote mental health and well-being
- Monitor clients’ progress and report any concerns or changes to supervisors
- Facilitate transportation to appointments, activities, or community events as needed
- Maintain accurate documentation of services provided and client interactions
- Promote safety and ensure compliance with all relevant policies and procedures
Qualifications
- High school diploma or equivalent required; additional certifications in social work or home health care related fields are a plus
- Prior experience working with vulnerable populations or in community services is required
- Strong interpersonal and communication skills with the ability to build trusting relationships
- Empathy, patience, and a genuine desire to help others succeed
- Ability to work independently as well as part of a team
- Valid driver’s license for transportation duties and a criminal background check.
- Ability to handle sensitive situations with professionalism and discretion
