As a Nurse Practitioner (NP) on contract, you provide advanced documentation support to our neurodiverse patients by completing the forms that secure access to the services, benefits, and accommodations they need to thrive. Your primary focus is the Disability Tax Credit Certificate (DTCC, CRA Form T2201), along with school, post-secondary, and workplace accommodations forms, and other government and private documentation.

- Excel at communicating with patients and families about what a form does, what is required to support it, and what to expect from the process.
- Can work independently and collaboratively with physicians, psychologists, and other healthcare professionals on the clinic team.

Responsibilities
- Complete Disability Tax Credit Certificate (DTCC, CRA Form T2201) applications for children and adults with neurodevelopmental conditions, ensuring submissions are clinically accurate, well-substantiated, and framed to maximize the likelihood of approval.
- Complete school, post-secondary, and workplace accommodations forms, including documentation for school boards, university and college Accessibility Services offices, OSAP Bursary for Students with Disabilities (BSWD), and employer accommodation requests.
- Complete other government and private forms as required, such as ODSP medical documentation, CPP-Disability applications, long-term disability and insurance forms, and private benefit attestations.
- Review patient records to gather the clinical information needed to support each form, and consult with the treating clinician where additional detail or clarification is required.
- Conduct focused interviews or chart reviews with patients and families to clarify functional impacts, daily living implications, and history relevant to each application.
- Track the status of submitted forms, respond to requests for clarification or additional information from CRA and other adjudicators, and assist with appeals, reassessments, and re-submissions where appropriate.
- Maintain meticulous documentation in the patient record, consistent with College of Nurses of Ontario standards, clinic policy, and privacy legislation.
- Stay current with eligibility criteria, form revisions, and adjudication trends so that submissions reflect the most effective and accurate framing for neurodevelopmental conditions.
- Other duties as mutually agreed upon.
- Virtual or from our Toronto office, as per your preference.
Qualifications
- Registered Nurse (RN) designation with the College of Nurses of Ontario (CNO).
- Nurse Practitioner certification with the CNO, including passing the NP examination.
- Experience completing disability, accommodations, and benefits documentation is strongly preferred. Familiarity with the DTCC (T2201) is an asset.
- Commitment to ongoing professional development and staying current with medical advancements relevant to neurodevelopmental conditions.
- Excellent patient and team communication, and adherence to ethical and professional standards in patient care.
Engagement
This is a contractor engagement. Hours are flexible and tied to clinic referral volume, with the option to work virtually or from our Toronto office at 1910 Yonge Street. Compensation is on a per patient referral basis to be agreed upon at the time of engagement.
Pay: $120.00 per hour
