Job Summary
We are seeking a compassionate and dedicated Community Support Worker to assist individuals in various healthcare settings. The ideal candidate will provide essential support to patients and families, ensuring they receive quality care and assistance in their daily lives. Duties
- Assist patients with daily activities such as bathing, dressing, and mobility to promote independence and comfort.
- Support medical staff by monitoring patient conditions and reporting any changes or concerns promptly.
- Administer basic medical tasks under supervision, including vital signs and medication reminders.
- Provide emotional support and companionship to patients and their families during treatment or recovery periods.
- Assist with case management activities, including coordinating care plans and communicating with healthcare providers.
- Maintain accurate documentation of patient interactions and care provided in accordance with facility protocols.
- Support patients in navigating healthcare services, including scheduling appointments and understanding treatment plans.
- Adhere to all safety protocols, infection control procedures, and confidentiality regulations.
Requirements
- Previous experience in hospital settings or nursing is highly desirable.
- Knowledge of acute care procedures, medical terminology, and case management practices.
- Experience with hospice care or pediatrics is a plus.
- Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare teams.
- Compassionate nature with a commitment to providing patient-centered support.
- Ability to work independently as well as part of a multidisciplinary team.
- Certification or training in basic life support (BLS) or CPR is preferred but not mandatory.
- Flexibility to work various shifts as needed to meet patient care requirements. Pay: $17.86-$29.37 per hour
